This notice describes our privacy policy and how we treat the information we receive about you.

As a fraternal benefit society, the Association of the Sons of Poland has always been committed to protecting the confidentiality and security of members. In this notice references to the Association of the Sons of Poland will use the terms *us*, *we*, and *our*.

When you applied for insurance we received information about you from your application, such as your name and address and certain health information. We treated the information we collected about you in a confidential manner. In all cases only the amount of information necessary to underwrite your policy has been collected. This information is used for business purposes with respect to insurance and our fraternal relationship with you. We do not disclose any nonpublic personal information about you to anyone, except as permitted by law.

Among the benefits recently offered is the prescription card and the contract with the company includes a confidentiality statement. As we expand the benefits we offer you, we may disclose nonpublic information to nonaffiliated third parties such as life insurers or mortgage bankers. Again, this information will only be disclosed as permitted by law. We do not disclose your personal information to companies or organizations not affiliated with us that would use the information we have provided them to contact you about their own products and services.

We restrict access to nonpublic personal information about you to only those employees who need to know information to provide products or services to you. We maintain physical, electronic and procedural safeguards that comply with federal and state regulations to guard your nonpublic personal information. We may disclose information about you in order for us to conduct our business, such as general administrative activities. Information may also be disclosed for audit purposes or to law enforcement and regulatory agencies.

Should your relationship with us end, your personal information will remain protected in accordance with our privacy practices as outlined in this notice.
